Sažetak
Pyoderma gangrenosum (PG) a rare ulcerating skin disease and a diagnosis of exclusion can be presented as another skin disease. Also, very often skin ulcers with a clinical resemblance to PG are misdiagnosed as PG. Cutaneous ulcerations in patients with suspected PG often prove, on further workup, to have a different cause. Moreover, treatment directed at PG — high-dose prednisone or other immunosuppressive medications — may be contraindicated in patients with any of several diseases that may produce ulceration resembling that of PG1 We report of a patient with a venous leg ulcer with severe pain that “mimic” pyoderma gangrenosum, which was treated with high doses of prednisone with no respond. Ultrasound of the leg revealed severe saphenofemoral insufficiency and wide, incompetent perforators. Prednisone was stopped and the patient was treated with multilayered compressive therapy and polyurethane foam dressings. Prompt healing of the ulcer was achieved in several weeks. Incompetent or insufficient perforator veins are often unrecognized but they are the most common cause of recurrent varicose veins after treatment. Perforator vein insufficiency can result in pain and ache that may be severe with bursting sensations, skin changes, and skin ulcers2
