Sažetak
INTRODUCTION Hypertension might be the most important public health problem as it impacts more than 26% of the global population and is responsible for 14% of deaths globally. In 2015 there were more than 20, 000/100, 000 cases of hipertension. Large scale of the problem, proper management and prevention of hypertension is necessary in order for prevention. METHODS Reviewing of relevant databases with a review of current research in the field of pharmacological approach to hypertension. RESULTS Recent reviews show that pharmacological and non- pharmacological interventions are necessary for effective control. Despite the availability of effective medications, blood pressure cannot be controlled in about 70% of patients who receive only pharmacological treatment. Recently shown, low patient adherence might be the cause of this phenomenon. To combat this it is important to promote a healthy and productive doctor-patient relationship to maximize patients adherence to anti- hypertensive drugs and long term lifestyle changes. The most common causes of poor drug adherence are side effects considered to be too high compared to the effect of treatment or administration drug challenges. Other causes are a lack of trust in conventional medicine and patients wanting to try lifestyle interventions and alternative medicine instead of pills. CONCLUSION Doctors are recommended to reveal barriers to the treatment and to inform the patient about the societal benefit of treating hypertension, as that might contribute to the patients understanding of the importance of treatment. Doctors are also recommended to modify treatment to facilitate increased adherence, with stand-out methods being using medications with tolerable or optimally no side effects. Drug monitoring can be seen by the patient as mistrust from the doctor so it is recommended to present drug monitoring as a control of treatment effect (as in some drugs with a small therapeutic window like dioxin or lithium) to promote the patient- doctor trust.
