Sažetak
Background: Non-dippers have a higher risk of developing cardiovascular diseases than dippers, regardless of blood pressure values. Objectives: To determine whether there are differences in basic laboratory findings and pulse wave velocity (PWV) between prehypertensive individuals depending on the dipping profile. Methods: This study included 35 patients (18 women, 17 men) with prehypertension according to the JNC7 criteria from 2003 (systolic blood pressure [SBP] 120-139mmHg or diastolic blood pressure [DBP] 80-89mmHg based on 2 or more measured seated measurements on each of 2 or more office visits) from one family medicine practice in Health Center Zagreb West. Basic laboratory tests (cholesterol, creatinine, fasting glucose, and potassium), anthropometric, 24h ambulatory blood pressure, and PWV measurements were done in all patients. Renal function was calculated with the CKD-EPI formula (KDIGO 2020). Patients were divided into two groups depending on a dipping profile (blood pressure drop during the nighttime more or less than 10%). Mann-Whitney U test and descriptive statistics were used in data processing in Statistica v.12. Results: A total of 24 patients were dippers (68%) while 11 patients were non-dippers (32%). Non- dippers were older (median 58 vs. 55 years ; p=0.05) and had lower DBP (median 69 vs. 72mmHg ; p=0.028), significantly worse renal function (median 71 vs. 100ml/min/1, 73m2), higher LDL cholesterol (median 4.5 vs. 3.3mmol/L ; p=0.019) and higher PWV (median 7.3 vs. 8.5m/s ; p0.001). No differences were found in the concentration of potassium, fasting glucose, triglycerides, or body mass index (median 24kg/m2). Conclusion: This pilot study indicates the possible existence of accelerated atherosclerosis in prehypertensive individuals with a non-dipping profile compared to those with a normal dipping profile. It is necessary to conduct research on a larger sample to confirm these findings.
