Sažetak
Introduction: Dialysis parameters are used for prescribing the dose of hemodialysis (HD) therapy (prescribed Kt/V). Delivered HD dose (Kt/V) value is calculated by various formulae utilizing pre- and postdialysis blood urea concentrations. Technical hemodialysis adequacy is measured by calculating the ratio of delivered and prescribed Kt/V (Qe). In order to demonstrate and analyze predictors of Qe values, we analysed a group of hemodialysis sessions. Subjects and methods: Data was processed on 132 dialysis treatments involving a group of 23 chronic HD patients. Results: There were differences between dialyses in females and males in prescribed Kt/ V (1.51±0.15 vs. 0.97±0.12, p=<0.001), delivered Kt/V (1.26±0.24 vs. 1.14±0.21, p=0.004), and Qe (0.84±0.16 vs. 1.18±0.22, p=<0.001). The strongest and negative correlations were found between Qe and urea concentration after HD (â = -1.059, <0.001) and between Kt/V Daugirdas and urea concentration after HD (â= -1.427, p=<0.001). Delta values (difference between prescribed and delivered HD dose) were correlated to urea concentration before HD (â =-0.436, p=0.001) and urea concentration after HD (â=0.388, p=0.007). Male gender and volume distribution were highly positively related to the Qe values (r=0.674, p=<0.001 ; r=0.640, p=<0.001). Between two groups of patients (with delivered Kt/V >1.2, and Kt/V <1.2) we demonstrated differences in urea concentration after HD (8.28±2.13 vs. 11.39±2.88, p<0.001), ultrafiltration volume (3.12±1.09 vs. 3.55±1.24, p=0.036), dry BMI (24.43±3.58 vs. 26.19±2.98, p=0.003), Kt/V UKM (1.32±0.29, 1.13±0.29, p<0.001), and Qe (1.09±0.25 vs. 0.94±0.25, p=0.001). Conclusion: We propose delta value as a dialysis technical efficacy variable. Delta values less than 1.0 have to be explored.
