Sažetak
Abstract text Background: During the last two years we have been challenged by the COVID-19 pandemic which significantly affected global health care systems’ ability to provide services to patients. Considering that, health care providers rapidly adapted to the use of telemedicine to make patients safe and less exposed to coronavirus infection. Case presentation: We present a 70-year-old woman who is the only person in Croatia so far to donate a kidney and then require chronic haemodialysis treatment followed by kidney transplantation (TX). She has been regularly vaccinated against SARS- CoV-2. The TX was performed in August 2020. Post- operative follow-up was conducted, with a gradually decreasing frequency of medical check- ups. After the TX she was hospitalized several times due to complications, such as deep vein thrombosis and acute pyelonephritis. Regularly, she contacts her nephrologist in the office by phone and they discuss her symptoms or difficulties. If the nephrologist assesses that a face-to-face examination is not necessary, on the day of check-up in the outpatient clinic located in the TX hospital, she takes laboratory exams and immediately thereafter leaves the hospital area. A physical examination is performed if one is indicated by symptoms discussed over the phone. Medical personnel sends her nephrologist’s finding with results of laboratory exams by e-mail. She returns to the clinic only in case her condition worsens. This way she is minimally exposed to COVID-19. She is also allowed to contact a nephrologist by phone, message or e-mail in case of any emergency. When visiting a hospital outpatient TX clinic is not required, the patient undergoes examinations in her local laboratory. Conclusion: So far, it has been shown that telemedicine has improved the medical approach, currently having a protective role against the new coronavirus infection, but traditional physical examination is in certain situations irreplaceable.
