Sažetak
The purpose of this study was to evaluate detection of different patterns of diffuse infiltrative lung disease in conventional chest radiographies. Independently interpreted lung HRCT was used to detect the presence of lung interstitial tissue pathology. Based on the HRCT findings of the corresponding lung tissue, conventional x-rays were considered normal (169 images), with a reticular (40 images), nodular (22 images), or a reticulonodular pattern (21 images). Four experienced radiologists independently examined the randomised and coded set of 252 chest x-rays. The procedure was repeated after more than two months. Three examiners showed similar sensitivity and specificity in detection of normal or abnormal lung interstitial tissue, while the remaining examiner detected more interstitial pathology but with less specificity and overall accuracy. All four examiners showed similar reproducibility of their findings (A: 82.54%, B:75.00%, C:73.41% and D:73.81%). It is possible that described differences reflect personal experiences and education of examiners. All four examiners had difficulties in detecting nodular patterns (sensitivity <20%) and two examiners in detecting reticulonodular patterns. It seems that early signs of nodular, or reticulonodular interstitial patterns are hard to detect on chest x-rays. In patients with dyspnea and no evident pathology on chest x-rays, HRCT of lungs should be considered.
