Sažetak
Introduction: Lichen sclerosus (LS) and lichen planus (LP) are chronic inflammatory mucocutaneous diseases with malignant potential that can coexist in the same patient. A good coordination among gynecologists, oral medicine practitioners, dermatologists and pathologists might enable early diagnosis, effective management of these lesions and better quality of life. Case presentation: A 57-year-old woman was referred to the Department of Oral Medi- cine, School of Dental Medicine, University of Zagreb due to the oral lesions that caused burning, pain and discomfort. Examination of the oral cavity revealed erythematous le- sions along the upper gingiva and lace like striations present on the buccal mucosa bilat- erally. A biopsy sample was taken and histopathology showed chronic inflammation, with some elements that could be incorporated into the diagnosis of lichen. She was referred to the gynecologist (Polyclinic Aviva, Zagreb) due to her nine months history of white patches on the vulva accompanied with pruritus, burning and dyspareu- nia. Gynecological examination revealed diffuse white polygonal papules that coalesced into plaques at the inner part of the labia majora. The patient was referred for biopsy to the Department of Gynecologic Oncology, Clinic for Gynecology and Obstetrics, Clini- cal Hospital Center Zagreb. Histopathological examination was conclusive for lichen scle- rosus. Patient was further successfully treated with clobetasol propionate 0.05% oint- ment for five months. Conclusion: There is often a delay in diagnosis of vulvar LS and oral LP due to lack of awareness in patients as well as physicians. A long-term follow-up of these patients is rec- ommended.
