Sažetak
Primary Sjögren’s syndrome (pSS) is an interesting autoimmune disorder (AID) of unknown etiopathogenesis in the light of complex neuroimmunoendocrine dysregulation in genetically vulnerable individuals, exposed to environemental factors. Current paradigm of immunopathogenesis of AID based on the reductionistic approach – the isolation of prestressed triggers is confronted to the multifactor scope of the disorders which is complementary to their heterogenic nature and diagnostic procedure. Considering the variety of clinical and immunology manifestations of the pSS as a model of systemic and local autoimmunity with the potential of early atherogenesis (AG), evolu- tion into other AID and malignant alterations – multidisciplinary approach in the evaluation of neurohumoral regulation, the role of the polymorphism of genetic markers and early AG is required. Scientific interest is based on the assumption of immunopathogenic connection between pSS and AG ; combined with epidemiological data about SS as the second most frequent AID (after RA) and the atherosclerosis as a main cause of death in modern world. Interaction of serotoninergic system with humoral and cellular immunity and other biomarkers of the systemic and the local autoimmune reaction in pSS at the same time modulate the activity of inflammatory mediators of AG. The polymorphism of genes which regulates serotoninergic system of serotonin transporter (SERT, 5-HTTLPR, 5- HTTVNTRin2), AG (MTHFR-Ala222Val, PPARG- Pro12Ala), as well as production of cytokines involved in the regulation of Th1/Th2 (responsible for cellular immunity/humoral immunity) and Toll- like receptors (TLRs) regulation and expression together with other predicitive biomarkers are still the focus of our scientific research. Multidisciplinary approach in the research, diagnostic and evaluation of neurohumoral regulation of autoimmunity and atherogenesis in pSS will contribute to the formation of “network” of institutions relevant for the diagnosis and monitoring of patients with pSS in field of the basic and the clinical medicine, to introduction of new diagnostic methods, selection of patients and approaching to a potentialy new immuno- modulatory therapy.
