Sažetak
Objective: Preopertive autologous blood donations(PABD) reduce or even remove the risk of exposure to allogenic blood. It is frequently used in adult and pediatric patients who undergo elective surgery, but it is used more cautiously in elderly patients, because they usualy have series of coexisting diseases. Study design and methods: A retrospective study was made in elderly patients, older than 65 yrs at Department of Orthopedic Surgery from 1999. to 200I. Their responses to PABD program were evaluated by analysis of incidence of donor reactions to autologous phlebotomy, operation type and demants for allogenic blood. Results: Between January 1999. and December200I, 10II patients participated in PABD at Department of Orthopedic Surgery: 663 were under 65 yrs and 348 (34, 4%) were over 65 yrs. 1032 units (l, 5unitJpt) of autologous blood were drowen in patients under 65 yrs and 536 units (1, 5units/pt) in patients over 65 yrs. Only 7 (2%) patients over 65 yrs had vasovagal reactions accompained by vasomotor instability. Of the 348 patients over the age of 65 yrs, 196 (56%) had primary total arthoplasty ; 88 (25%) had primary total kneearthroplasty ; 48 (14%) had revision type total hiparthroplasty and 6 (2%) patients had no surgery due to organizational problems. Two hundred sixteen (62%) patients received only autologous blood ; 121 (35%) patients also required allogenic blood and 5 (I%) did not require blood transfusion at all. Conclusion: Preoperatove autologous blood donation is feasible and effective procedure in majority of the elderly patients undergoing elective orthopedic surgery.
