Sažetak
Background:Preoperative autologous blood donation is commonlyperformed to meet potential perioperative transfusion needs and isa particularly common prior to hip and knee arthroplasty.Aims:Weanalysed the relationship between predonation hemo-globin levels, as well as other factors as age, gender, weight, andtype of total joint replacement surgery, and the need for postoper-ative blood transfusion.Methods:Weretrospectively analysed 417 patients having surgeryduring 2004: 288 (69.06%)with primary total hip arthroplasty(pTHA) and 129 (30.94%) with primary total knee arthroplasty(pTKA). 184 women (63.8%) and 104 men (36.2%) were included inpTHA group. The median age was 61 (range 26–79 year), the medianweight was 77 (range 45–120) kg. The median predonation hemo-globin (Hb) was 139 (range 111–173) g/L and median predonationhematocrit (Htc) was 41 (range 34– 54)%. 108 women (83.7%) and21men (16.3%) were included in pTKA group. The median age was66 (range 38–79) years, the median weight was 80 (range 50–114)kg. The median predonation Hb was 138 (range 119–168) g/L andmedian predonation Htc was 41 (range 34–49)%. Patients from bothgroups predonated in average 1, 4 (range 1–2) units of autologousblood.Results:Patients in both groups required a transfusion therapy. 244(84.7%) patients with pTHA were transfused only with autologousblood and had average predonation Hb of 14 093 (SD 11.73) g/L.Other 44 (15.3%) patients who required autologous and homolo-gous blood, had average predonation Hb level of 13 509 (SD ±9.01)g/L. We found a significant relationship between the need for post-operative transfusion and the predonation Hb level (P=0.003), pre-donation Htc values (P=0.006), weight (P=0.003) and gender (P=0.002): female patients and patients with lower predonation Hband Htc, as well as patients with lower body weight more oftenneeded additional homologous blood transfusion. No relationshipwas found between age of patients and the need for transfusion (P=0.121). 104 (80.6%) patients with pTKA were transfused withautologous blood only, and had average predonation Hb level of 13 867 (SD ±9.06) g/L. Other 25 (19.4%) patients transfused withautologous and homologous blood had average predonation Hblevel of 13 588 (SD ±8.93) g/L. The significant relationship wasfound between the need for postoperative transfusion and weight(P=0.012): patients with lower body weight more often neededadditional homologous blood transfusion. No relationships werefound between predonation Hb level (P=0.099) predonation Htcvalues (P=0.243), gender (P=0.241) and age (P=0.276) of patientsand the need for postoperative transfusion.Conclusions:Our results show that over 80% of patients neededonly autologous blood. In our patients with pTHA predonation Hbwas significant predictive factor for additional transfusion therapy, while in pTKA it was not observed. In both groups of patients bodyweight was significant predictive factor, thus this feature seemsimportant for planning of transfusion therapy in patients with pTHAand pTKA.
