Sažetak
This study provides an overview of approaches to purchasing health services. The main basic approaches were identified as fee-for-service, fixed budget, diagnosis-related groups, capitation payment and outcome-based payments. Since a single approach cannot be considered a general solution, each of the approaches and related factors were explored in detail. The level of overall financing of healthcare as a percentage of GDP varies among EU countries and it is an important factor. The fee-for-service approach is mostly used in outpatient settings and generally increases the volume of service, while it does not control for quality or efficiency of the service provided. The fixed budget approach puts the healthcare provider in charge of managing its patients carefully, taking into consideration the quality of service and possible complications that could produce additional cost, as well as the current needs of the patients. Due to the immediate pressure and limited (insufficient) funds, this approach likely focuses on acute, life-threatening conditions and neglects patients who ‘can wait, ’ treating them as future problems, thereby generating long waiting lists. The diagnosis-related groups pay an average cost for each patient, thereby encouraging the provider to be more efficient in the delivery of service. Outcome-based payment methods are discussed as a solution for innovative drugs.
