Sažetak
The progressive improvement of blood transfusion safety has led to remarkable decline of interest in preoperative autologous donation. However, if future would show that immunomodulation induced by allogeneic blood transfusion is clinically significant, and if shortages of blood would continue, the interest for preoperative autologous donation (PAD) will rise again. Primary total joint replacement is a common surgical procedure associated with considerable blood loss and usage of blood products. In order to improve blood transfusion practice in this group of patients, we performed prospective reviewing of requests for transfusion and we organized clinical rounds with surgeons and anaesthesiologists on a monthly basis. In this report we have shown blood transfusion practice in 1, 217 patients. Over the four-year period, proportion of patients enrolled in PAD increased (47 % vs. 70 %), as well as a proportion of patients who were transfused with autologous blood only (17 % vs. 57 %). Overall exposure to blood decreased (3.2 units vs. 1.8 units per patient). Criteria should be established to monitor the appropriateness of autologous transfusions. Transfusion of autologous blood without clinical indication increases the risks of adverse reactions and the cost of medical care. Collaboration between clinicians and transfusion service physicians will decrease wastage of autologous blood and improve autologous blood program.
