Sažetak
Medullary carcinoma of thyroid gland grows and infiltrates neck and mediastinal structures. Sometimes it interferes with the airway causing obstruction of trachea. Airway management is essential skill of each anesthesiologist. To be skilled in different methods is essential for ENT anesthesiologist because a new patient gives new pathology and a need for a new approach. It is important to admit that you maybe will not succeed and to have a help standby. Case report: A male patient, 69 years old, weighing 62 kg, with a medullary carcinoma of a thyroid gland spreading into neck and mediastinal structures obstructing trachea. He was presented to the ENT unit. ENT surgeons, thoracic surgeon and abdominal surgeon performed total thyroidectomy, total laryngectomy, radical neck dissection (right), sternotomy, mediastinal lymphadenectomy and jejunostomy. Discussion: Fiberoptic intubation is maybe the safest approach in securing airway but sometimes flexible instrument is not suitable to bypass tracheal obstruction. Rigid endoscope permits maintaining direction and placement of an ET tube bypassing obstructed trachea.
