Sažetak
A 70-year old man with history of alcohol consumption and history of head trauma for the past 3 hour, arrived at our Emergency Department. Neurological examination revealed severe headache, without motor and sensory disturbances and without clinical signs of intracranial hypertension. Glasgow Coma scale (GCS) was 14/15. His general condition was good, in addition to being mildly confused, with elevated blood ethanol levels. Emergency head computed tomography (CT scan) suggested an epidural hematoma. A CT scan showed a significant left tempo-parietal epidural hematoma, it was typically bi-convex (or lentiform) in shape, and hyperdense, somewhat heterogeneous, and sharply demarcated. (Figure 1. A, B and C) Acute epidural hemathoma was considered suitable for surgical evacuation. Emergency surgery was performed to remove the intracranial hematoma, and the patient underwent the surgery. Our patient died in spite of decompressive craniectomy with evacuation of the hematoma, due to complications. He developed a hemorrhage postoperatively. The duration of postoperative coma correlated with the morbidity rate in survivors. Acute epidural haematomas remain among the most common causes of mortality and disability resulting from traumatic brain injury. Although epidural hematomas are relatively uncommon (less than 1% of all patients with head injuries ), it should always be considered in evaluation of a serious head injury. However, acute epidural hematoma may also occurs after mild or moderate TBI
