Sažetak
Intracapsular temporomandibular disorders are a group of functional disorders that occur from anatomical abnormalities or anomalous positions of the temporomandibular joint structures. Disc displacement with and without reduction, degenerative joint diseases and subluxation are considered to be disorders that affect the internal structures of the temporomandibular joint. If an individual is unable to properly adapt to intracapsular changes, a painful condition, known as arthralgia, might develop. Some intracapsular disorders require no treatment, while others can be treated with one of many options available – including occlusal splints. Occlusal splint therapy is a reversible treatment modality used for temporomandibular disorders in individuals with arthralgia of the temporomandibular joint, degenerative joint diseases and disc displacement with reduction, as well as treating patients affected by nocturnal parafunctions. There are two types of occlusal splints that are most commonly used: the stabilization splint and the anterior repositioning splint. As the name says, the stabilization splint stabilizes the whole masticatory system by removing an existent orthopedic instability between the occlusal position and joint position. It is used in patients with arthralgia, degenerative joint diseases and those suffering from nocturnal bruxism. On the other hand, the anterior repositioning splint is used in patients with disc displacement with reduction to position the mandible in a more forward and anterior position to enable a better relationship of the condyle-disc complex to the articular fossa. Impressions of the upper and lower jaw, face bow, centric relation and protrusion/laterotrusion records need to be transferred to a dental laboratory for occlusal splint fabrication. The fabrication process can happen in one of three ways: by flasking, acrylate layering or digitally, which is followed by a trying-on and adaptating the finished product.
