Sažetak
Affective or mood disorders, as the most common in the general population, are the largest group of psychiatric disorders. According to the World Health Organi-zation, the incidence and prevalence of these disorders have reached „epidemic” levels. The prevalence of depressive disorder varies between 2 and 10%, while lifetime risk of developing illness is 20-30%. The prevalence of bipolar disorder is about 1%, although in newer studies it reaches up to 6%. Since 1898, these disorders have been classified in Krepelin’s dichotomy in a categorical approach. Recent studies have tried to inaugurate the dimensional approach to the concept of both disorders, especially in the distinction between „normal” and clinical de¬pression. In clinical work, the challenge is to differentiate clinical depression from normal sorrow as well as to diagnose bipolar disorder. In a bipolar depressive epi¬sode there is no specific indicator to discriminate the first episode of recurrent de¬pression and the depressive phase of bipolar affective disorder, which is crucial for proper treatment. The treatment of both disorders is comprehensive and relates to the paradigms of the biopsychosocial concept and the individual approach. Diagnosis and treatment of mood disorders have a long tradition in Croatia. The history of treating these disorders started with the application of the first tricyclic antidepressants in the 1950s. As previous studies were mostly orientated towards schizophrenia and alcoholism, there was a limited number of Croatian psychia¬trists dealing with mood disorders. Over the past decades, there has been a trend of organizing departments and institutes in major psychiatric centres according to diagnostic categories, which allows psychiatrists to be oriented in a specific area. Today, diagnosis and treatment of mood disorders in Croatia are at a high level and performed according to global standards. Thanks to integrated treatment, a significant decrease in suicide rates has been achieved, and we are successful in rehabilitating our patients, as majority of them recover and return to their working and social environment.
