Sažetak
Background:Asthma is a syndrome characterized by airway obstruction that may vary spontaneously or due to medicamentous effects. Aim is to determine the consumption of antihistaminics and antiasthmatics in the City of Zagreb, and to provide guidelines for rational prescription of these agents on the basis of the results obtained. Methods: Data on drug consumption were obtained from 16 pharmacies, members of the Zagreb Municipal Pharmacy, evenly distributed all over the city territory. These data were extrapolated to the Zagreb total population (data provided by the National Bureau of Statistics). Drug consumption was calculated from the number of prescription issued at primary health care level, and by tracing data on total number of packages used, financial parameters, and number of DDD/1000inhabitants/day as a standard measure for drug consumption monitoring in ATC system. Results:During 2001, total consumption of systemic antihistaminic (ATC R06A) in Zagreb was 25.81 DDD/1000/day, with the greatest consumption of loratadine (R06AX) of 20.41 DDD/1000/dan. In the total consumption of antiasthmatics of 9.95 DDD/1000/day, the inhalatory adrenergic salmeterol (R03AC) prevailed with 3.14 DDD/1000/day, followed by the anticholinergic ipratropium (R03BB)with 2.82 DDD/1000/day and glucocorticoids (R03BA) with 1.37 DDD/1000/day. Conclusion: The prevalence of allergic diseases, especially asthma, has been on a steady increase in Zagreb and Croatia as a whole, however, comparison with some neighboring countries, e.g. Hungary, revealed an almost twofold consumption of antihistaminic in the City of Zagreb. Thus, the question arises of the justifiability of such a wide use of these agents. In contrast to Hungary, where salbutamol still predominates in antiasthmatics therapy, in Croatia the increasing consumption of salmeterol and ipratropium as more efficacious and less harmful agents appears to be justified in spite of their higher cost. This holds for their use in children in particular, as a population group most comonly affected with astma.
