Sažetak
Aim: To preserve the bundle bone by extruding a tooth planned for extraction with magnets to achieve an aesthetically satisfactory restoration. Materials and methods: A 35-year-old patient presented with a fracture of the tooth 21 in the cervical region. The tooth had been previously endodontically treated. Due to the impossibility of making an adequate prosthetic replacement, it was decided to extract the tooth in question and replace it with an implant. However, after performing a CBCT scan, a thin vestibular lamina dura and radiolucency in the apical region were observed, which would compromise the final outcome of implant therapy. Prior to extraction, the tooth in question was gradually extruded with neodymium magnets to achieve an optimal aesthetic result. Impressions of the maxillary and mandibular arches were used for wax-up of the missing tooth. The models were then used as a base for fabrication of a transparent splint. Two 2 mm magnets were used for root extrusion - the first was anchored in the root canal and fixed with flowable composite, while the second was fixed in the splint with condensable composite. The distance between the magnets was 2 mm. The splint also served as a temporary replacement. The amount of extrusion was controlled every 14 days. After the magnets connected, the root was shortened by 2-3 mm and the magnet was fixed in a new position in the root canal. Results: A control CBCT scan showed a vertical increase of 2.6 mm in the vestibular and 0.9 mm in the palatal lamina dura after completion of 4 months of extrusion, which was sufficient for a satisfactory esthetic result. Conclusion: Although the indication for the use of neodymium magnets for extrusion purposes is very limited, in this case the magnets represented an excellent solution to maximally improve the final outcome of implant prosthetic therapy.
