We investigated the impact of antibiotic resistance on the treatmentof blood stream infections (BSI). During two separate month periods (1-30.6. and 1-30.11.1999.) we followed all blood culture isolates in nine Croatian hospitals. There were 32 982 hospital admissions in the study period with 154 episodes of clinically significant bacteremias (4.7/1000 patients) 70.1% of which were community acquired. In these patients dominant isolates were enterobacteriaceae (72%) with antibiotic sensitivity as follows: ampicillin 48, co- amoxiclav 80, cefuroxim 90, ceftriaxon 94, ceftazidime 95, ciprofloxacin 99, gentamicin 98, amikacin 99, imipenem and netilmicin 100%. There were 11 S.aureus isolates, all sensitive to oxacillin, four penicillin sensitive pneumococci, and four ampicillin sensitive enterococci. Enterobacteriaceae were also the most common causative agents of hospital acquired BSI (32%), followed by non-fermentors (22%), S.aureus (16%) and enterococci (12%). Gram-negative isolates showed sensitivy to antibiotics as follows: co- amoxiclav 18.5, cefuroxim 25.9, ceftriaxon 25.9, ceftazidime 44.4, imipenem 88.9, gentamicin 51.9, amikacin 70.4, netilmicin 74.1, ciprofloxacin 74.1%. Among S.aureus isolates 75% were resistant to methicillin, but all were sensitive to vancomycin. All enterococci were sensitive to ampicillin and vancomycin. Empirical therapy was adequate in 78% of community acquired and 15% of hospital acquired BSI.
