Sažetak
The aim of the study was to evaluate the role of high resolution ultrasonography (US) associated with fine needle aspiration biopsy (FNAB) in the detection of local recurrence of differentiated thyroid carcinoma. Neck ultrasonography was performed in 340 patients with prior thyroidectomy and radioiodine ablation of the thyroid remnant. Enlarged lymph nodes and/or mass in the thyroid bed were found in 274 patients. US guided FNAB was performed in 98 patients with sonographic features suggesting malignancy. The malignant cells were found in 32 patients who had tumor tissue varied in dimensions from 8x6x6 mm to 25x19x15 mm. Nine patients had recurrences in the thyroid bed. Five of them had lymph node metastases as well. Twenty three patients had just lymph node metastases. Radioiodine scintigraphy was negative in 27 of 32 patients with recurrences in the thyroid bed and/or neck lymph node metastases. All patients with no evidence of tumor tissue at US guided FNAB had negative 131-I whole body scintigraphy. Twenty four of 32 patients with malignancy had elevated serum Tg levels (>1ng/mL). Our study indicates that US associated with FNAB should be performed during the follow-up of patients with differentiated thyroid carcinoma regardless of the results of other diagnostic methods.
