Sažetak
Riedel's thyroiditis is a rare disease of unknown etiology characterised by invasive fibrosclerosis of the thyroid gland that often involves surrounding tissues. Many reports of extracervical fibrosclerosis associated with Riedel's thyroiditis suggest a systemic nature of the disease. Other reports reveal an autoimmune etiology and connect Riedel's thyroiditis with Hashimoto's disease and other autoimmune disorders. Surgical treatment is often unsatisfactory, and therefore is usually combined with steroid treatment. A few reports showed an effective treatment of Riedel's thyroiditis with tamoxifen. A 46-year old female presented with clinical diagnosis of Riedel9s thyroiditis, previously diagnosed Hashimoto's thyroiditis and hypothyroidism. A disease had progressive course with one-year duration. At the time of admisson to our Department the patient had dyspnea, dysphagia, fatigue and hoarseness. She was previously treated with small doses of steroids prescribed a year ago because coexisting autoimmune disorder and occasional pain in the neck. The patient was treated with tamoxifen, small doses of steroids, L-thyroxin and followed up during one year. Treatment with tmoxifen showed a significant subjective impprovement and objective changes confirmed by periodic clinical examinations, cervical ultrasound and computed tomography imaging. The patient has no compression symptoms and goiter decreased in size from 105 grams to 63 grams (calculation based on ultrasound measurement). We are now considering the patient for surgical treatment and definite histopathologic confirmation of diagnosis. Our report confirms tamoxifen as valuable drug therapy in the treatment of Riedel's thyroiditis
