Sažetak
In this report we described an unusual case of glioblastoma which showed the features of granular cell change. The aggressive tumor composed mainly of granular cells and also containing anaplastic small glial cells was studied histologically and immunohistochemically. Granular cells contained cytoplasmic PAS, diastase-resistant granules. Glial fibrillary acidic protein was detected in minority anaplastic glial cells and in marginally plac! ed cytoplasm of some of the granular cells. Vimentin was diffusely demonstrated in both cellular populations. The granular cell component was positive for S-100 protein, CD 68, and alpha-1-antichymotrypsin. Based on our light microscopic and immunohistochemical findings, the granular change is designated transformed glial cells and their prognosis is dictated by the glial component. It is important to recognize this histological variant of glioblastoma, particularly on a small biopsy sample, in which diagnostic confusion with an infarction, other anaplastic glioma or metastatic carcinoma is possible.
