Sažetak
The patient complained of difficulty chewing due to distinct grade III mobility of tooth 21. Small intraoral radiographic image of tooth 21 showed vertical bone resorption, while clinical examination found the presence of periodontal pockets of 6 mm depth on the palatal and approximal sides of the tooth, and gingival tissue recession of 3 mm on the vestibular aspect of the tooth. Clinical examination found a lack of support zone in lateral regions of the jaw, resulting in a reduction of the prosthetic plane level and disorder of intermaxillary relationships. This resulted in a revers overlap of frontal teeth, except tooth 21 which was due to abnormal migration caused by the loss of supporting structures, extruded from the arch. This presented functional and aesthetic disturbances for the patient. Following endodontic treatment of tooth 21 and initial periodontal therapy, we decided to restore tooth 21 proper position within the arch using orthodontic elastics anchored to the adjacent teeth combined with a temporary lifting bite using composite stoppers. In two days, tooth 21 was brought into the desired position, and immobilization with Ribbond tape was made, which also serves as a permanent retainer. Following splinting of the tooth, initial periodontal therapy II was made. Recall appointments after 3 and 6 months show a decrease in periodontal probing depth of about 3 mm. Recession of gingival tissue around the tooth 21 remained unchanged compared to the base-line and there were no signs of inflammation. The patient was extremely satisfied with the functional and aesthetic outcome of the conducted therapy. This case report showes that interdisciplinary approach in treating periodontal disease is necessary in some cases in order to achieve functional and aesthetic harmony on general patient satisfaction.
