Sažetak
HFNOT is increasingly used as part of both ward-based and critical care management of respiratory failure. Respiratory failure is distressing for patients and treatment modalities currently in use may be associated with discomfort from upper airway drying, tightly fitting facemasks, and resultant complications such as skin breakdown. Invasive ventilation is also associated with a number of complications including ventilator-associated pneumonia. HFNOT is very versatile and consistently better tolerated by patients than NIV. The advantage of this, apart from patient comfort, is that the patient can probably remain connected to the device for longer periods. The HFNOT seems more effective than conventional oxygen therapy and non-inferior to NIV in most studies. In recent years, HFNOT is gaining popularity as respiratory support of adult patients with acute hypoxemic respiratory failure or respiratory distress syndrome (ARDS), with respiratory compromise induced by heart failure, with respiratory compromise post-extubation and immediate pre-intubation period in intensive care, to improve gas exchange post-abdominal and cardiac surgery, and to facilitate bronchoscopy. Also it is a useful adjunct during airway instrumentation (especially when managing difficult airways), for immune compromised patients, and as a means of reducing suffering at the end of life. The triad of humidity, compliance, and high FIO2 that HFNOT offers is likely to be of use in a wide variety of clinical situations.
