Sažetak
We wanted to investigate the occurrence of depression in Croatian women with urinary incontinence (UI) in regard to the type and duration of UI. Depression is a condition of low mood and animosity to activity that can affect a person's thoughts, behavior, tendencies, feelings, and sense of well-being. It is associated with person’s perception of helplessness, lack of control, and inability to cope with various problems and affects several levels of personality, causing changes in both mental and physical functioning. Women with UI have a higher prevalence of depression compared to women of the same age without UI. Most studies report that depression is present in 44% of women with UUI and 17.5% of women with SUI, contrary to nearly 6% of the general population. Possible explanation for the difference in rates of depression found in women with and without UI is that women with UI suffer from depression because they isolated themselves from society and because they are embarrassed, ashamed and anxious about involuntary leakage of urine. This was a cross-sectional study of the occurrence of depression among women with three common types of UI: stress UI, urgency UI and mixed UI. The study was carried out from March 2017 through July 2018 in Department of urology University Hospital Center Osijek in Croatia. The total analyzed sample of women in our study was 120. Women were divided into three groups comprised of 40 subjects (33.3%) according to the aforementioned three types of UI. The mean age was 57 years (interquartile range 49 to 64 years) ranging from 29 to 82 years. The state of psychological health was assessed with the help of a Becks Anxiety Inventory and Becks Depression Inventory. On the Beck's scale, anxiety median score was 15 (interquartile range 7 to 22) and depression median score was 12 (interquartile range 9 to 20) without any significant differences in relation to the type of UI. According to the Beck’s scale low-anxiety was present in 85 patients (70.8%), moderate in 27 (22.5%) and potentially worrisome anxiety was present in 8 patients (6.7%), with no significant difference compared to e type of UI. In UI symptoms that lasted longer the level of anxiety was higher (Spearman's coefficient of correlation ρ = 0.290 P = 0.001) and also the level of depression was higher (Spearman's coefficient of correlation ρ = 0.347 P <0.001). UI and depression are interrelated. The study of the mental health status of women with UI should be included in the regular assessement in the Urology outpatient clinics for the evaluation of psychosocial factors that could reduce the symptomatology of UI. In order to improve the interaction between the patient and the physician, further studies are needed to evaluate optimal approach to the patient's clinical examination through validated questionnaires in the Urology outpatient clinic.
