Sažetak
Results from the present studies shows that 1 of 4 patients who were discharged from the hospital after acute myocardial infarction will experience myocardial infarction, stroke or cardiovascular death, in the period of next 5 years. Risk for the one of this event is extremely high during the first year, but it could be decreased with the secondary prevention treatment and the participation in the cardiovascular rehabilitation program. One can define cardiovascular rehabilitation as a program in which polyvalent medical team use interdisciplinary and transdisciplinary approach to encourage actively and give a support to the cardiovascular patient to reach and keep optimal physical and psychosocial health and to improve the duration and the quality of life. Beside the individual approach depending on the risk stratification, for optimal success of program it is necessary to actively include patient's partner or family members. Cardiovascular rehabilitation program continues on the acute hospital treatment and it is implement in the institutions which are adequately equipped with diagnostics and therapeutic tools. All members of the rehabilitation team must be always ready for emergency situations. In the Republic of Croatia, like in other European countries, cardiovascular rehabilitation is done at the hospital (21 days ; Special Hospital for Medical Rehabilitation Krapinske Toplice, and Special Hospital for Medical Rehabilitation Thalassotherapia Opatija) or in outpatient clinic (3-5 times/week during 3 months ; Institute for Cardiovascular Prevention and Rehabilitation, Zagreb). During the last decade in high developed countries part of the cardiovascular rehabilitation program starts in a patient's home with the use of information and communications technology according to the telecardiology principles. The cardiovascular rehabilitation program include medical gymnastics under the supervision of cardiologist with telemetric ECG monitoring, complete diagnostics elaboration of cardiologists and physiatrist, psychodiagnostic assessment and psychological advices, therapeutic education, nonpharmacological treatment and pharmacological therapy. Depending on comorbidities, other specialists are consulted – psychiatrist, endocrinologist, vascular surgeon, neurologist etc. Participation in cardiovascular rehabilitation program has following goals: A) medical – improvement in the myocardial function, decreasing the risk of sudden cardiac death, reinfarction, disappearance of symptoms (dyspnea, chest pain), improvement in working ability, prevention of the progress of atherosclerotic process ; B) psychological – improvement of self-confidence, decrease of anxiety and depression, stress management, return of sexual health ; C) behavioral – smoking cessation, healthy diet, regular body exercise, taking a medicines ; D) social – return to work if it is possible or to previous functional capacity, independence in everyday activities ; E) for heath system – decreased cost of treatment, early mobilisation, early hospital discharges, decreased rehospitalisation because of cardiological symptoms.
