Sažetak
Introduction: An explicit European list of potentially inappropriate medicines (PIMs), the EU(7)-PIM list, was recently developed in order to analyse and compare prescribing patterns across European countries 1. Due to multiple co- morbidities with subsequent polypharmacy, and altered drug pharmacodinamic and pharmacokinetic, older people are at an increased risk of experiencing adverse drug events, frequently as a consequence of PIMs 2. Aim: The aim of this study was to measure the prevalence of potentially inappropriate medicines (PIMs) among older patients by using the EU(7)-PIM list and to ascertain their association with adverse clinical outcomes. Materials and methods: A prospective observational study was conducted with acutely or electively admitted elderly patients to the Department of Internal Medicine, Dubrava University Hospital between October 2014 and April 2016. PIMs were identified by using the EU(7)-PIM list as an identification tool. Clinical outcomes were obtained following three to six-month hospital discharge and included the incidence of hospital readmission and emergency department visits, symptoms of the disease, adverse drug reactions (ADRs) and therapy amendments. Results: In 243 patients included in the study (male: 122 (50.2%), median age: 73 (range 65-92), median number of medicines: 7 (range 1-17), median co- morbidities: 7 (range 2-17)), a total of 275 PIMs were identified in 159 (65.4%) patients at hospital discharge. The most prevalent PIMs were benzodiazepines, followed by proton pump inhibitors and tramadol. A total of 53 (21.8%) patients had adverse clinical outcomes as a result of their therapy, but in only 10 (4.1%) patients adverse clinical outcomes (n = 11) were associated with identified PIMs. Most of the PIM-related outcomes pertained to ADRs (n = 9), while the symptoms of the disease accounted for two clinical outcomes. Conclusion: The new EU(7)-PIM list detected a high prevalence of PIMs among the elderly, albeit a poor relation between PIMs and relevant clinical outcomes.
