Sažetak
Introduction: Urinary tract infections are very common in humans, with Escherichia coli as dominant pathogen. It is usual to treat patients with uncomplicated bacterial cystitis by empirical antimicrobial therapy, according to known local resistance rates. Resistance rates of amoxycillin (43, 41% ) and trimethoprim/sulphamethoxazole (24, 67%) in Split and Dalmatia County, Croatia reached the levels that excluded them as empiric choice in treatment of UTI. Quinolones, as norfioxacin, with resistance rate 9, 27% are next choice for empiric treatment of UTI and it is important to monitor resistance to other antibiotics within quinolone-resistant E. coli strains. Aim: The aim of this research is to determine resistance rates to cephalosporins among norfioxacin-sensitive and norfloxacin- resistant strains of E. coli and to find out if there is any differences between these two groups. Material and Methods: In this study all isolates of E. coli were collected from urine of symptomatic outpatients from January 1, 2005 to December 31, 2005 in Split and Dalmatia County, Croatia. Identification up to the species level was made by conventional biochemical methods and APl10E (bioMerieux, France). Antibiotic susceptibility to norfloxacin NOR and to oral representatives of three cephalosporin-generations (cefalexin CN, cefuroxim CXM, cefixime CFX) was made by disk diffusion method. Number of strains tested to CFX was lower than number of other strains because CFX was introduced in laboratory testing on April 1, 2005. Chi-square analysis was used for determination of statistical significance in antimicrobial resistance to cephalosporins among norfloxacin-sensitive and norfioxacin-resistant strains of E. coli. Results: Resistance rates to oral cephalosporines among NOR-S and NOR-R E. coli are presented in table. Conclusions: Resistance rates for quinolones are increasing rapidly and it is important to monitor therapeutic option for quinolone-resistant E. coli. Our results show that there was significant difference among resistance rates to first- and second- generations of cephalosporins in quinolone-sensitive and quinolone-resistant uropathogenic E. coli. Third generation cephalosporin cefixime showed no significant difference in activity among quinolone-sensitive and quinolone-resistant strains of E. coli. According to good in vitro results, this drug can be used for the treatment of UTI caused with quinotone-reslstant E. coli.
