Sažetak
Aim: The objective of this research is to show theorganisation and work of occupational health services (OHS)active in primary health care in Croatiain the period 1995 to 2017 and their activities regarding provision of preventive health examinations. According to occupational medicine annual reports, we analyzednumber of full-time activeand part-time active OHS. We also analyzed the overall number of preventive health examinations of workers andthe number of other types of health examinations(e.g. seafearers, B licence drivers, etc.). Also, we show the changes innumbers of health examinations provided by clinical medicine specialists, conducted for the purpose of diagnosing occupational diseases or evaluation of workability. Materials and Methods: Croatian Institute of Public Health (CIPH) is a national public health institution that gathers all the datarelevant for giving the insight into the state of health of the nation. We analyzed the CIPH data gathered from the OHS and published in Croatian Health Statistics Yearbooks, covering the period of 1995 to 2017. Results are presented in absolute numbers, percentages and average annual percent change for trend analysis. Results and Conclusions: According to occupational medicine annual reportsfor the period 1995 to 2017, the average number of full-time active teams in OHSwas 159, rising constantly till 2013 (min. 137 in 2003, max. 194 in 2013) and the number of part-time active teams in OHS decreased. Annual number of preventive health examinations of workersincreased by 4, 13% (average annual percent change) ; minimum number of health examinations was 122.171 in 1999 ; and maximum number was 335.196 in 2015. Annual number of other preventive health examinations in OHSdecreased. Consultativehealth examinations are on a steady decline and the number of preventive health examinations is on the rise, having the 46% share in overallnumber of preventive health examinations at the beginning of the observed period, continuing in 2017 up to 65%. Annual number of examinations forevaluation of workability decreased by 7, 68% ; min. 290 in 2017 ; max. 6.429 in 1995, average 1.534.There were 83.764 diseases and conditionsdiagnosedin workers having health examinations in OHS in 1995, and by the year 2017 the number has increased by three times (min. 83.764 in1995, max. 305.743 in 2015).Diagnosed diseases and conditions are encoded by ICD-10, where exposure to noise and vibration is also continuously observed. These data are often analyzed with caution because there is always one small part of the OHS that does not submit their annual work reports. Comorbidities can not be shown here because aggregated data. The most common external causes of the disease found in the OHS is exposure to noise at the workplace, followed by exposure to vibration. Almost during the entire observed period they make up for more than a half of the external causes of the disease. It is therefore very important to stimulate co- operation between County institutes of public health and primary health care offices and increase the quality of annual reports so that analyzes can be made from these data for the purpose of better health care planning.
