Sažetak
Contact allergic dermatitis (CAD) and irritant dermatitis (CD) on the face, and comparative diagnoses of rosaceiform dermatitis (RD), seborrheic dermatitis (SD) and atopic dermatitis (AD) were followed up during 18-month period at the Allergy and Cosmetics Unit, Department of Dermatology, Zagreb, University Hospital Center Zagreb, Croatia. The study included 142 patients. In all patients, patch (epicutaneous) test was applied in accordance with the International Contact Dermatitis Research Group (ICDRG) recommendations. The patients were also submitted to humoral immunity testing (prick test) for inhalant and alimentary allergens, and to scratch test for preserva- tives and additives, that were also analysed for the above diagnoses. There were patients with applications of topical steroids (hydrocortisone, alclomethasone, betamethasone, flumethasone, clobetasol), and various neutral moisturizing creams and lotions. In 124 out of 142 patients was also analysed Pityrosporum ovale-mycosis - in 115 (92.74%) of them were positive results. In 108 out of 142 patients was analysed parasite Demodex folliculorum and 58 (53.70%) were positive. These all were treated with topically antimycotic or antiparasit creams. The patients were submitted to patch testing for delayed type of sensitivity, and prick and scratch testing for immediate type of allergy. The patients were divided in the several diagnostic groups - CAD - 68 patients (47.89%), CD - 5 patients (3.52%), RD - 15 patients (10.56%), SD - 16 patients (11.27%) and AD - 2 patients (1.41%), however, some of these patients had two diagnoses: CAD + SD - 23 patients (16.20%), CD + SD - 6 patients (4.23%), CAD + RD - 4 patients (2.82%), RD + SD - 2 patients (1.41%) and CAD + AD - 1 patient (0.71%). The diagnoses were differentiated on the basis of clinical evaluation and test result analysis.Typical clinical picture, long term treatment for facial dermatitis, and results of patch testing pointed to allergy on irritation on the face.
