Sažetak
OBJECTIVE: To determine disease incidence and epidemiologic characterictics of infection cause by Streptococcus pyogenes among school children in Zadar area. METHODS: we retrospectively surveyed medical records from Epidemiologic unit and Gsneral Hospital from 1982 to 1992 Also, we conducted a seroeoepideiologic survey to identify the strains of particular pathological importance.RESULTS: A total of 5880 children with proved streptococcal infection were analysed. The most frequent clinical disease was pharyngitis/ tonsillitis (78.4%) followed by scarlatina (21.3%). erysipelas (0.3%) and pyoderma. Two epidemics of acute pcslstreptococcal glamerulonophrits (APGN) with 38 cases and and 46 sporadic cases were registered over the study period. Rheumatic fever (RF) was noted only in one patient but eight cases of poststreptoccocal reactive arthiritis (PSRA) were noted from 1990 to 1992. Of the 137 isolated streptoccoci in 1991 and 1992, 123 (89.8%) belong to S. pyogenes. out of which 110 (89, 4%) were M typable, 114 (92%) T typable, whereas 13 (10, 6%) were nontypable ; 83 (67.5%) of the isolates were OF positive while 40 (32.5%) were OF negative. The most frequently nephritogonic serotypes isolated were M4 (26.01%) and M12 (17.07%). potentially rheumatogenic (M3, M5) and invasive strains (M3, M5, M11, M12, T11/12) were identified too. CONCLUSION: No changes were noted among the common streptococcal disease. In spite of absence of RF fever the presence of rheumatogenic strains points to a high epidemiologiacal risk. The risk of APGN remains high. Appearance of a larger nunber of PSRA suggests the need for a more accurate diagnosis in regard to antibiotic prophylaxis. Finally, the presence of potential invasive strains emphasises the risk of disease forms that are more severe
