Sažetak
Cerebrospinal fluid (CSF) leaks from defects in the anterior cra¬nial fossa present a difficult diagnostic and management problem. The authors present a case of a 44-year-old female with a unilateral CSF-leak from the nasal septum. The patient denied ever receiving a trauma to the head or nose, but she had had two episodes of meningi¬tis. Computed tomography (CT) and nuclear magnetic resonance (NMR) imaging with cystemography strongly suggested a direct con¬nection between the endocranium and the unilateral tumefaction of the nasal septum. During endoscopic endonasal transseptal surgery, a cribriform plate was clearly presented, and the anterior skull base was sealed by fibrin glue and a piece of harvested nasal mucosa. There were no complications after the operation, and the patient is still free of symptoms fifteen months after the procedure. The origin of this extremely rare clinical entity remains obscure. The authors speculate that an arachnoidal cyst was the most probable background.
