Treatment of Varicocele at Paediatric age
Sažetak
A varicocele is an abnormal dilation of the pampiniform plexus of the testicular veins. Its most commonly developed during adolescence when the testicles grow dramatically, with incidence around 16%. Varicocele may alter testicular growth, spermatogenesis, and fertility potential. The pathophysiology of testicular injury is presumed to be related to increased scrotal temperature, but it's still poorly understood. The appropriate treatment of children and adolescents with varicocele should be individualized, based on the presence of risk factors predictive of long-Term subfertility. The main indications for surgical intervention remain significant left (≥20%) or bilateral testicular hypotrophy, pain or abnormal semen analysis, the latter most reliable in at least 18 years old boys. The most commonly used techniques for repair of adolescent varicocele are inguinal/subinguinal, suprainguinal retroperitoneal, open or laparoscopic approach with ligation of the internal spermatic vein. If the large varicocele is repaired during adolescence, the more likely is recovery of spermatogenic function and likelihood of pregnancy
