Sažetak
Introduction: Although the importance of breastfeeding is well known, the use of breast milk substitutes is widespread, starting in hospital. This practice negatively affects breastfeeding duration, which is associated with increased infant and maternal morbidity. Objective: To determine why supplementation is given to healthy, term newborns at the Department of Obstetrics and Gynecology, University Hospital Split, and whether mothers are informed and consent to this practice. Methods: This was a cohort descriptive study of 342 mother/infant pairs who gave birth at the Department of Gynecology and Obstetrics in Split from April 6 to July 30, 2011. Data were collected using Newborn Feeding Charts, made for the purpose of the study, in which everything given to the child on the maternity ward was recorded, and UNICEF questionnaires used for monitoring Baby-Friendly hospitals. Data were processed using descriptive statistics ; for categorical data and testing differences in proportion. T test for independent samples was used. Values were considered significant if P <0.05. Results: During the hospital stay 62.8% of infants received supplements. In 94.1% of cases, newborns were given artificial milk (formula) ; the rest received sugar water. The most common reasons for supplementing, according to the mothers, were: lack of milk (49.8%), a crying baby (35.5%), fatigue/pain due to cesarean section (11.5%), newborn weight loss (10.6%) and sore nipples (10.1%). Ninety- one percent of mothers were informed of the need for supplementation by hospital staff and their consent was sought ; 2.7% were notified but their consent was not sought and 5.9% were not notified prior to supplementation but found out later. Conclusion: Based on our study, it appears that mothers face numerous challenges with breastfeeding while in the hospital, indicating an urgent need for increased breastfeeding support. The finding that there were mothers who were not notified of the intention to supplement their newborns, indicates the need to improve communication between mothers and hospital staff.
