Sažetak
PURPOSE: To describe and analyze the causes of anterior knee pain on magnetic resonance imaging (MRI) in children and adolescents younger than 25 years of age. METHODS: The study included 100 patients with anterior knee pain at the age of 10-25 years. All patients underwent MRI of the knee joint, and the soft tissue and bone changes in the patellofemoral joint and the knee extensor apparatus were analyzed. RESULTS: Sixty-eight percent of patients were male. The median age was 19 years. In 52% of patients the changes of the knee extensor apparatus were found: of the proximal part of the patellar tendon in 26% of patients ; of the distal part of the patellar tendon in 21% of patients ; of the quadriceps femoris tendon and myotendinous junction in 5% of patient. In 18% of patients chondromalacia in patellofemoral joint was found, 6% of patients only had a thickened mediopatellar plica, and 5% of patients had thickened synovial fold and chondromalacia. Eleven percent of patients had lateral patellar dislocation, four of them with history of recent trauma. Patella bipartita was found in 3% of patients. In 2% of patients inflammatory changes of the synovium in suprapatellar recess was found. One patient had infrapatellar fat pad edema, one patient had pretibial bursitis and in one patient thickened infrapatellar plica was found. CONCLUSION: The most common causes of anterior knee pain in children and young physically active people under the age of 25 are changes of the extensor apparatus of the knee joint due to overuse, rarely as a result of acute trauma. In about one in five patients the articular cartilage lesions of the patellofemoral joint can be found, and in a small number of patients chondromalacia is associated with thickened synovial folds. Lateral patellar dislocation is not so rare. In a minority of cases anterior knee pain is caused by inflammation of the synovium and bursitis.
