Sažetak
The goal of our investigation was to examine complications in patients with acute necrotizing pancreatitis (Kummerle Ilird degree). A retro¬spective analysis in the ICU was conducted. Patients were admitted to ICU in the state of shock (SBP<90 mmHg). CT of pancreas indicated necrosis of the pancreas together with necrosis and inflammation of adjacent structures. Thirty-six patients were treated, 22 men and 14 women, of an average age of 49±10 years. APACHE II score on admission was 14.53±5.45, median 16, and Ransons's score was 3.44±0.91, median 3. Complications during the treat-ment were cardiovascular (100%), ARDS (27 patients or 75%), renal (29 or 80.6%), hepatic (26 or 72.2%), hematological (24 or 66.7%), and neurological (3 or 8.3%). MODS was observed in 29 (80.6%) patients. All patients developed extensive cardiovascular^side effects, and a high percentage damages of other organs. Frequency and severity of complications could be reduced by timely monitoring of global and regional circulation and other vital parameters, as well as by the application of all required treatment procedures.
