Sažetak
Human transferrin receptor (TfR) is a transmembrane dimeric glycoprotein expressed at the cell surface on nearly all cell types, in particular, in rapidly proliferating cells with special iron requirements, such as erythropoietic cells of the bone marrow. Serum soluble transferrin receptor (sTfR) is a proteolytic product of the intact TfR and as a monomeric fragment of the extracellular domain circulates in the blood. The concentration of sTfR correlates directly with erythropoietic activity and inversely with the amount of iron available for erythropoiesis. Consequently, changes in sTfR concentration have been observed in hematologic disorders associated with alterations in erythropoiesis and/or body iron status. In recent years a number of authors have investigated the diagnostic significance of sTfR in differential diagnosis of anemias. We have performed a study to evaluate the clinical significance of sTfR and sTfR/logF parameters in anemias of various types: iron deficiency anemia, anemia of chronic disease (rheumatoid arthritis) and anemia in chronic renal failure. The results showed that sTfR and sTfR/logF index levels were significantly higher in patients with iron deficiency anemia than in controls and patients with anemia of chronic disease and anemia in chronic renal failure. These results showed sTfR and sTfR/logF to be reliable parameters in the diagnosis of iron deficiency anemia, as such as in the differential diagnosis of iron deficiency anemia and anemia of chronic disease. In patients with anemia in chronic renal failure sTfR and sTfR/logF parameters were significantly higher in patients receiving recombinant human erythropoietin (rHuEPO) therapy in relate to patients without rHuEPO therapy. The results suggest that sTfR measurement could be a reliable parameter to monitor erythropoiesis in the prediction of the hematologic response to rHuEPO treatment.
