Sažetak
Sympathetic hyperactivity is linked to resistant hypertension (RH), target organ damage and cardiovascular comorbidity. RH, the inability to achieve goal blood pressure despite the use of three or more appropriately dosed antihypertensive drugs, including a diuretic, is a common problem. In RH patients it is very important to check for abnormalities of renal arteries. Multiple renal arteries (MRA) accounts for most common kidney anatomic abnormality. Second most common is fibromuscular dysplasia, with frequency of 2-6.6%. Stenosis of the main renal artery and its mayor branches accounts for 3-6% of hypertension. The common cause in the middle-aged and elderly is an atheromatous plaque at the origin of the renal artery, in younger is heterogeneous group of lesions termed fibromuscular dysplasia. The aim of the present study was to estimate the prevalence of MRA in RH patients. The results of our study suggest that RH patients should be carefully screen for secondary cause and abnormality of renal arteries. MRA accounts for most common kidney anatomic abnormality. The mechanism of developing hypertension in patients with MRA is still unknown. Further investigations are necessary to determine connection between MRA and RH.
