Sažetak
For many years a "wait and see" strategy with postponed psychopharmacotheray has prevailed in the treatment of first episode psychosis. Both the "prevention strategy" and the "wait and see" approach have their pitfalls. A pitfall of the prevention strategy includes the possibility that patients not fated to develop psychosis may be unnecessarily treated for a long time with antipsychotics. On the other hand, the "wait and see" strategy may lead to losing precious time an appearance of "full-blown" psychosis with all its consequences. The clear differentiation between the prodromal and the premorbid phase as well as between the prodrome and frank psychosis needs to be elucidated. New strategies are needed to increase the accuracy of recognition of the outpost syndrome or"at risk mental state" for first episode psychosis as well as to increase treatment effectiveness.
