Sažetak
Adenomyoepithelioma is a rare breast tumor and an unusual type of dimorphic tumor, characterized by the presence of both epithelial and myoepithelial cells. Adenomyoepithelioma is arare breast tumor and an unusua ltype of dimorphic tumor, characterized by the presence of both epithelial and myoepithelial cells. We present the case of a72 year-old woman to whom a breastnodule is detected during the breast cancer screening. A mammogram showed a shadow with an indistinct margin, and directed US image showed an irregular 7 mm size, hypoechoic lesion with fuzzy margin, wich was suggestive of a malignant tumor. An ultrasound-guided FNA biopsy was performed. The aspirated specimens showed rich cellularity in the form of large tightly cohesive clusters composed of spindle cell with oval nuclei, fine chromatin, inconspicious nucleoli and scanty bipolary located amphophilic cytoplasm. Clusters of spindle cells were associated with fragments of metachromatic stroma (MGG). There were clusters and sheets of benign apocrine cells admixed with clumps of spindle cells. We.concluded that this tumor was of mesenchymal origin, without the presence of malignancy. Immunostain performed retrospectively on the cytological smears, with antibody to smooth muscle actin (SMA) showed positivity in some cells in clumps. Pathohistological diagnosis was adenomyoepithelioma. The spindle or polygonal cells were positive for myoepithelial markers (SMA, S-100) and the epithelium was positive for epithelial marker (GCDFP-15) by immunohistochemistry. To aid the appropriate diagnosis of adenomyoepithelioma in a FNA cytology careful examination of the MGG smears as well as immunocytochemistry are very helpful.
