Sažetak
Intensive treatment policy needed to induce remission in haematological patients resulted in disruption of host mechanical defence barriers against various microorganisms and disturbed host cellular defence too. Infectious complications are a dominant cause of attributable morbidity and mortality among haematological patients. Therefore dealing with infections in treatment of haematological patients is a continuous challenge. Incidence of bacterial infections in these patients mostly depends on the level and duration of induced neutropenia as well as mucosal damage and the usage of indwelling central venous catheter. On the other hand immunosuppressive therapy and suppression of T-cell mediated immunity is largely the reason for putting patients at risk of fungal, viral or protozoan infections. In most studies the rate of infectious complications reported amounts up to 90 %. Documented bacterial infections accounted for 30 to 40 % of febrile episodes. One third of patients had a fever of unknown origin. Improved control of bacterial infection led to increasing of invasive fungal infections. Invasive fungal infections are associated with high rates of mortality in haematological patients. In light of this facts prevalence of invasive isolates from blood and bronchoalveolar lavage in patients treated at Department of haematology of Clinical Hospital Merkur is discussed.
