Transcranial color-coded duplex (TCCD) sonography represents an alternative to current methods used as confirmatory tests to evaluate cerebral circulatory arrest. In the process of brain death confirmation, transcranial Doppler (TCD) was proven to be a reliable method of assessment of cerebral circulatory arrest (CCA). TCCD sonography is a safe, non-invasive bedside procedure but it has only been exceptionally used to establish cerebral circulatory arrest. This report is a prospective study based on a small series designed to determine the feasibility of TCCD sonography utilisation in monitoring cerebral hemodynamic status and CCA for confirmation of brain death. Transtemporal, transforaminal and transorbital approaches were used with B-mode imaging of intracranial anatomical structures, in addition to color-coded flow images. Ten comatose patients were ultrasonographically monitored. Six patients with a Doppler pattern of cerebral circulatory arrest underwent computed tomography angiography (CTA). Oscillatory flow or systolic spikes were found by all approaches in five patients. In one patient, no flow was detected by the transtemporal approach, but oscillatory flow was detected in the basal arteries by transorbital approach. In all six patients the diagnosis of CCA was consequently confirmed by CTA. The TCCD technique could be a valuable alternative method for evaluation of cerebral circulatory arrest. It represents a safe and inexpensive technique that can be performed at the bedside to shorten and simplfy the diagnostic process of CCA. The transorbital approach is a useful addition which improves the performance of this distinct Doppler technique.
