Blood units for transfusion therapy must not have clinically significant red blood cell (RBC) antibodies (Abs). According to the literature data, clinically significant in vivo destruction of patient’s RBCs will occur only after transfusion of at least one unit plasma with clinically significant RBC Abs with titer more than 128. Because of that, weak positive reactions (score < 5) in sensitive micromethods do not refer to the presence of RBC Abs with the potential of causing RBC destruction in blood recipient. Aim was to point out specificity and serological activity of RBC Abs in blood donors with RBC Abs, and to estimate the posibility of changing algorithm for confirmatory testing performing in glass beads technology of blood donors in CITM. We examine 294 blood donors withRBC Abs from the CITM database from 2001 to 2010. RBC Abs screening was performed by Solidscreen II (Bio-Rad Laboratories, Dreieich, Germany). Confirmatory testing was done in microcards by gel technology (ID-Card Bio-Rad Laboratories, DiaMed GmbH, Cressier FR, Switzerland). After negative results in gel technology, according to CITM algorithm, confirmation was done in glass beads technology (Bio Vue System, Ortho Clinical Diagnostics, Raritain, USA) following manufacturer’s package inserts. From 294 blood donors, 24 had clinically insignificant RBC Abs, 117 had RBC Abs with possible clinical significance and 153 had clinically significant RBC Abs. Among them there were 94 (32%) blood donors with RBC Abs of scores < 5 in glass beads technology and negative in gel technology. Those blood donors which were in immunohematological testing of RBC Abs reactive in glass beads technology were in third of cases non reactive in gel technology. Respacting the significance of each donated blood unit, we introduced gel technology as a standard for confirmatory immunohematological testing of blood donors in CITM.
