Sažetak
Introduction: A considerable interest in Helicobacter pylori infection and its consequences for mankind has been seen since the beginning of the paste decade in different countries all over the world. The bacterium has also drawn attention from scientist in central and eastern Europe has become the subject of numerous scientific investigations carried in central and eastern Europe dates back to 1986 and 1987, it was only in October 1992 that investigators from these countries interested in H. Pylori were given the opportunity to meet together for the first time. The meeting devoted to H. Pylori reaserch and trends in gastrodudenual disorders in eastern European countries were organized in Prague by the European Helicobacter pylori Study Group and permited exchange expiriences in H. Pylori research field among approximately 100 scientist from Albania, Bielorussia, Bulgaria, Croatia, Checz Republic, Estonia, Hungary, Lithuania, Poland, Romania, Russia, Slovakia, Slovenia and Ukraine. The data presented at this meeting made many of the participants realize the importance of H. Pylori infection and its possible consequences for these countries.
To avoid confusion over management of H. Pylori infection the Croatian Helicobacter pylori Study Group was founded in 1995. Following the Mastricht Consensus report, and guided by clinical experience in one hand,and drugs available on the Croatian market CHPSG established recommendation fro H. Pylori treatment.
Recommendation: Helicobacter pylori has been known to play major role in the etiology of gastritis (type B), and infection with H. Pylori is present in almost all patients with duodenal ulcer, in 90 percent of patients with gastric ulcer, in patients with gastric cancer (H. Pylori has been classified as a definite carcinogen by the Internationa Agency for research in Cancer), and patients with MALT lymphoma.
Respecting the fact that H. pylori eradiction remarkably decreases the possibility of ulcer relapse and apperarance of disease complications, and subsequently treatment costs, and guided by the local data on H. Pylori resistance, clinical efficacy, availability and prices of drugs in Croatia, as well as with Maastrich Consensus Guidelines, the CHPSG recommended the following one-week triple thearapy schemes appropriate for H. pylori eradication:
amoxicillin 1000 mg bid, azithromycin 1g/3 days, omeprazole 20mg bid or pantoprazole 40mg bid;
amoxicillin 1000 mg bid, metronidazole 500mg bid, omeprazole 20mg bid or pantoprazole 40mg bid;
azithromycin 1g 3 days, metronidazole 500mg bid, omeprazole 20mg bid or pantoprazole 40mg bid.
Second-line therapy:
azithromycin 1g 3 days, doxicycline 100mg bid or oxytetracycline 500mg bid, omeprazole 20mg bid or pantoprazole 40mg bid.
