Sažetak
Aim: The objective of our study was to evaluate the diagnostic accuracy and limitations of fine-needle aspiration cytology (FNAC) in the diagnosis of Hodgkin’s lymphoma (HL) and anaplastic large cell lymphoma (ALCL) as well as its potential of distinguishing these lymphomas. Methods: We analysed a total of 116 consecutive patients with an initial FNAC diagnosis of HL and/or ALCL during a period of seven years (2004–2010) as well as the corresponding histologic reports. We also reviewed the immunocyto/histochemical markers (CD15, CD30, CD20, ALK, EMA, CD3, EBV-LMP, PAX5, CD4, MUM-1), clinical features, treatment and survival of these patients over a period of 8 years and 5 months (1 January 2004 – 1 June 2012). The patients’ age ranged from 16 to 93 years (mean ± SD: 37.8 ± 17.17 ; median: 33 years), 84 patients were diagnosed in the early clinical stage of the disease (I–II), 32 were diagnosed in the late stage (III–IV) and 53 patients had B symptoms. Results: The initial FNAC diagnoses included 86 cases of HL, 26 cases of ALCL and four cases in which a determination between the two entities could not be made (HL/ALCL). In the HL group a comparison with the histologic reports was possible in 81 cases and in 93.8% the histologic diagnosis was HL. In 3.7% it was ALCL and in two cases it was non-ALCL non-Hodgkin’s lymphoma (non-ALCL NHL). In the ALCL group the same comparison was made in 23 cases and, according to histology, 43.5% were diagnosed as ALCL, 39.1% were diagnosed as HL and four cases were classified as non-ALCL NHL. Since we had an almost uniform group of patients, according to therapeutic approach, we performed univariate analyses and found that patients younger than 55 years, in the early stage of the disease and without B symptoms had significantly longer overall survival (OS) (P < 0.05). Also, according to the results of FNAC analysis, a diagnosis of HL and ALK negativity was significantly associated with longer OS (P < 0.05) whereas, concerning histology, we found no significant difference in OS between patients diagnosed either as HL or ALCL. On the other hand, positivity for CD15 and EMA negativity was significantly associated with longer OS (P < 0.05). While narrowing the analysis to 98 patients which were diagnosed either as HL or ALCL (according to both FNAC and histology) we found no significant difference in OS between these two entities. This could be due to the conspicuous similarities in clinical behaviour of these diseases or possibly because of the small number of patients in our study, thus calling into question the clinical relevance of distinguishing these two lymphomas. We feel that a larger study is necessary to fully address this issue. Conclusion: Our results indicate that FNAC is a significant, reliable and very accurate diagnostic method, especially in HL. Furthermore, FNAC may have clinical relevance as a method of distinguishing HL from ALCL and vice versa.
