Sažetak
Babezioza je česta i potencijalno fatalna multisistemska bolest pasa uzrokovana intraeritrocitnim protozoama iz roda Babesia koje prenose krpelji iz roda Ixodidae.Uzročnik babezioze pasa u Hrvatskoj u najvećem je broju slučajeva Babesia canis canis koja je sposobna uzrokovati varijabilnu kliničku sliku, s brojnim životno ugrožavajućim komplikacijama. Budući da proupalni i protuupalni odgovor nositelja dosada nisu bili prikazani na jednom te istom uzorku cilj ovog istraživanja bio je ispitati njihov međusobni odnos koji je odgovoran za razvoj SIRS-a, MODS-a kao i niza komplikacija ove bolesti. U istraživanje je bilo uključeno ukupno 97 pasa, 72 psa oboljela od babezioze i 25 zdravih pasa, od kojih je u 10 pasa apliciran antibabezijski lijek Imizol®. Za svakog pacijenta utvrđena je prisutnost SIRS-a, pojedinačnih komplikacija i MODS-a, te su posljedično, s obzirom na stupanj bolesti, svrstani u četiri skupine: nekomplicirani SIRS/sepsa, komplicirani SIRS/sepsa, tešku sepsu i septični šok. U svih je bolesnih pasa ustanovljen razvoj SIRS-a. Bolesnim psima i zdravim psima kojima je apliciran Imizol® praćeni su tijekom istraživanja hematološki i biokemijski parametri, osmolalnost seruma, te koncentracije hormona štitnjače, inzulina, kortizola, natrija, kalija, PCT-a i NT-pCNP-a u šest navrata.Ovim su istraživanjem ustanovljene statistički značajne pozitivne korelacije proupalnog medijatora NT-pCNP-a s protuupalnim hormonima inzulinom i kortizolom, inzulina, kortizola NT-pCNP-a i osmolalnosti s ishodom, a statistički značajne negativne korelacije hormona štitnjače s inzulinom i kortizolom te hormona štitnjače s ishodom. Koncentracija PCT-a nije korelirala ni s koncentracijom NT-pCNP-, niti s koncentracijama kortizola i inzulina iz čega proizlazi da PCT nije dobar prognostički biljeg ni oblika niti ishoda babezioze u pasa.

Canine babesiosis is a multisystemic protozoal, potentially fatal disease. In Croatia babesiosis is mostly caused by intraerithrocytic piroplasm named Babesia canis canis and it has a wide range of clinical signs which can result in various outcomes, from mild to fatal.The differences in clinical manifestations of babesiosis appear to be the result of the interplay of the parasite-host interactions.Since proinflammatory and anti-inflammatory response in canine babesiosis was not yet studied in the same model, the main aim of this study was to investigate the interplay of the proinflammatory and anti-inflammatory response, and their influence on the development of SIRS, MODS, and series of other complications.Among 97 dogs that were included in this study, 72 had babesiosis, and 25 were healthy. To ten healthy dogs antibabesial treatment (imidocarb dipropionate) was administered as a preventive measure on the request of their owners. Haematology and biochemistry profile, as well as the concentrations of T4, fT4, insulin, cortisol, PCT, NT-pCNP and osmolality were determined for each dog. SIRS was present in 100% of patients. Considering severity of the disease dogs were classified in four different groups: uncomplicated SIRS/sepsis, complicated SIRS/sepsis, severe sepsis and septic shock. This research registered the significant difference between values of electrolytes and hormones measured, as well as the osmolality between healthy dogs and those with babesiosis. In addition, concentration of electrolytes, T4, fT4 were significantly lower, and concentrations of insulin, cortisol, NT-pCNP and PCT as well as the osmolality were significantly higher in different groups of sick dogs, mostly expressed in those with severe sepsis and septic shock. Moreover, the significantly positive correlation of NT-pCNP with insulin and cortisol, as well as insulin, cortisol, NT-pCNP and osmolality with outcome were ascertained. Significantly negative correlation was noted between thyroid hormones with insulin and cortisol, as well as between thyroid hormones and outcome.
