Sažetak
INTRODUCTION: Introduction of primary percutaneous coronary intervention (PCI) in the urgent treatment of ST-elevation myocardial infarction (STEMI) decreased mortality rate of cardiogenic shock, which was about 80% in patients treated conservatively. Authors report characteristics of STEMI in patients with cardiogenic shock and mortality rate in those treated conservatively and in those with primary PCI. MATERIALS AND METHODS: Among 734 patients hospitalized with STEMI in Coronary Care Unit, University Hospital “Sestre milosrdnice”, Zagreb, Croatia, 64 (8, 7%) of them developed cardiogenic shock, in 31 STEMI was treated conservatively and in 33 with primary PCI. RESULTS AND CONCLUSION: Patients with cardiogenic shock, in comparision with whole group of patients with STEMI, are on average older, with higher percent of female, and higher percent of inferior and expecially right ventricular myocardial infarction. In PCI group of patients with cardiogenic shock, in comparison with whole PCI group of patients with STEMI, culprit leasion is more often in right coronary artery. However, in patients who died as result of cardiogenic shock, anterior wall of left ventricle and left anterior descendent coronary artery are more often affected. Finally, mortality rate of cardiogenic shock in patients treated with primary PCI is much lower in comparison with data from literature and here discovered data about conservatively treated patients (36% vs. about 80%).
