Sažetak
Breast cancer (BC) is the most prevailing malignancy among female population and the major cause of mortality of women between the 40 and 50 age of life. BC represents genotypically and phenotypically very heterogeneous clinical entity, divided into numerous pathological subgroups whose clinical presentation, management, prognosis and treatment of choice, differ with respect to pathohistological features, tumor grade and size, lymph node involvement and expression of prognostic molecular markers [estrogen receptors (ER), progesterone receptors (PgR) and HER-2/neu protein expression]. The purpose of this study was to determine relationship between Her-2/neu, PgR and ER status, tumor grade and size, lymph node status and patients age in ductal invasive breast cancer female patients in Croatia. Methods: A total of 1645 cases of ductal invasive breast cancer diagnosed between 2005 and 2007 in Department of Pathology University Hospital for Tumors, Zagreb, Croatia were included in this study. All cases were graded according to the modified criteria of Bloom and Richardson, as described by Elston and Ellis. The hormone receptors were evaluated immunohistochemically using monoclonal antibodies against ERα (clone 1D5, Dako) and PR (clone PgR 636, Dako) while HER- 2/neu status was determined immunohistochemically (HerceptTestTM, Dako) and with chromogenic in situ hybridization (CISH) using Zymed SPoTTM HER2 CISH kit (Zymed inc., South San Francisco, CA, USA). The results were correlated with clinical and histological data such as patient’s age, tumor grade, size and axillary lymph node status. Statistical analysis of data was performed using Statistica 5.0 (StatSoft inc., Tulsa, USA). A p< 0.05 value was considered to be significant. Ductal invasive breast cancer occur among elderly postmenopausal patients (median 61 years) as ER positive (73.0%) and HER-2/neu negative tumor (83.1%) with less aggressive behavior (grade II) while more agressive ER negative and HER-2/neu positive tumor is more prevalent in younger patients. We also find that in younger patients ER, PR and HER2/neu negative tumor with worst prognosis (so called triple negative) occur in 21.1% while in elderly patients it is 11.4%. Results of statistical analysis show that in whole population Her-2/neu positively correlate with tumor grade and positive LN and negatively correlate with age of patients and hormone receptor status. Hormone receptors negatively correlate with tumor grade and positive LN. Yet, in younger age HER2/neu negative tumor is only relevant prognostic marker for LN metastases. Relevant prognostic markers for worse outcome of elderly patients are negative hormone receptors, positive HER2/neu and higher tumor grade.
