Sažetak
Corneal grafting is a method of choice for treating the patients loss of the vision caused by disturbances of the clear cornea. Although success rate reaches over 90% in uncomplicated cases (nonvaskularized leukomas, degenerative disorders), risk for graft failure rises sharply (65%-70%) in patients of a high-risk group (previous graft failure, vaskularization of the graft bed). The most important features for the maintenance of the dynamic, physiological process of the ocular immunological privilege include, integrity of the blood-ocular barrier, absence of HLA class II molecules in the central cornea and phenomena called ACAID. Apart from the rules for the most of solid organ transplantation there is not precise guidelines for the use HLA matched corneal tissue. In our study we observed 52 patients: Low-risk group n=28, High-risk group n=24. All the patients and donors were tissue typed with PCR on the HLA-DRB1* gen. Corneal transplantation was made according to the medical diagnosis. Keratopathia n=15, Leucoma n=10, Keratoconus n=7, Re-KPP n=6, Impending perforation n=4, Combustio corneae n=3, Degenerative disorders n=2, Keratoglobus n=1, Keratouveitis n=1, Corneal maculae n=1, Corneal melting syndrom n=1. The graft rejection frequency was higher in the group of high-risk patients (29%) than in the group of low risk (7, 1%). Graft rejection rates were not significantly higher in unmatched patients of both groups, high risk (31%) and low risk group (5%). Comparing rejection of compatible 37% for high risk, and 2% for low risk group, with the rejection rate of incompatible 44% in high risk and 5% in low risk group we confirmed our result that HLA-DRB1 matching does not improve corneal graft survival.
