Sažetak
Introduction ADRs are known to cause problems both in hospital and general practice settings, contributing to longer stays in a hospital, higher costs of hospitalisation1, 2. Patient factors are known to be associated with ADRs3, and patients who have experienced an adverse drug event (ADEs) have been shown to want information about their drugs4. In addition, we know that there is a relationship between desire for information and ADEs5. The aim of this study was to explore all the factors influencing this relationship to establish whether patients who intrinsically desire more information (high scorers) identify ADEs more readily than those who don't. Method Patients on five general medical wards at a London teaching hospital were interviewed at their bedsides using a standardised questionnaire: patient demographics ; scales to measure desire for information and anxieties about illness and open questions. The Naranjo algorithm was included to confirm the patients' experiences of an ADE6. The quantitative data were analysed using the Statistical Package for the Social Sciences (SPSS) version 10.0 and the qualitative data was analysed using QSR NUD*IST. Results and discussion A total of 120 interviews were successfully completed, 56.7% men and 43.3% women. The mean age was 63.5 years. Most patients had cardiovascular (44.2%) or respiratory (27.5%) disease as a primary diagnosis. The patients were prescribed an average of 7.7 medicines. Of those interviewed, 46.7% reported having an ADE in the past and 20.0% reported having an ADE during the hospital stay. Opioid analgesics accounted for 24% of the reported ADEs during the hospital stay. Younger patients tended to want more information about their medicines and scored higher on the EID scale. Previous experience of an ADE was found to correlate with age: younger patients had experiences ADEs to a higher extent than older patients, which may be confounded by a new ADE experience or by greater desire for information. Contrary to previous work, the EID score did not correlate with previous experience of ADEs in this study. "Physical characteristics" (age and ethnicity) and "social and socio-economic factors" (marital status) were related to the experience of an ADE. "Feelings and psychological factors" were not found to be associated with ADEs, nor was "illness and illness perceptions". Conclusion Further exploration is required to establish causality: whether patients who intrinsically desire more information identify ADEs more readily than those who don't, or whether the experience of an ADE increases a patient's desire for such information.
